Is It V Tach? Is an Urgent Diagnosis Necessary?

Wide-complex tachycardias (WCT) are common presentations in the emergency department. The differential includes ventricular tachycardia or supraventricular rhythms with aberrant conduction, a pre-existing bundle branch block, or conduction via an accessory pathway. Many electrocardiographic findings have been reported to accurately diagnose v tach, including several multi-step algorithms (Brugada and Vereckei). Often in the acute management of WCT an exact diagnosis of the origin of the rhythm is still not possible, and one must rely on clinical algorithms, in particular ACLS recommendations.
