It is recommended that public-access defibrillation (PAD) program for patients with out-of-hospital cardiac arrest (OHCA) be implemented in communities with individuals at risk for OHCA. Evidence for PAD and its effect on outcome from OHCA will be reviewed. There are many challenges to improve strategies of PAD program. The key challenge of PAD program is how to link willingness lay responders to public access defibrillators. Smartphone technology has the potential to amplify the effect of volunteer first responders and public-access automated external defibrillators (AED). Challenges and different strategies to overcome those challenges of PAD program will be reviewed.
